Do I need a referral from my insurance company?
Contact your insurance company and ask if they require a referral or authorization. If so, please bring the paperwork with you or call us to find out if your insurance company has sent it to us. How do I set up my first appointment?
After determining whether you need a referral or authorization from your insurance company, call our scheduling team at (239) 333-1177, or contact us by e-mail.
What forms do I need to fill out prior to my first appointment with Pain Management Consultants?
After scheduling your first appointment, you can open, print and complete theNew Patient Forms, and bring them with you. Please note you will need Adobe Acrobat to open and print the forms. Alternatively, you may call Pain Management Consultants at (239) 333-1177 and we will send you the forms via U.S. mail.
What should I bring with me on my first appointment?
Your completed New Patient Forms; any medical reports, documentation or radiology results (X-rays, etc.) that your primary physician may have provided you; health insurance card, if applicable; and driver license or other government-issued identification.
When should I arrive for my appointment?
If you have completed the New Patient Forms, please arrive at least 10 minutes before your scheduled time. If you have not completed the paperwork, please arrive 30 minutes prior. If you have not completed the New Patient Forms prior to arriving for your first appointment, be sure to bring a complete list of medications and dosage, if applicable. If you’ve had surgery, please write down what the procedure was and date of surgery.
What if I am late for any appointment?
If you arrive more than 15 minutes late, chance are you will have to re-schedule for another day so that we may continue to provide the best possible service to those who have arrived on time.
How do I pay?
Pain Management Consultants accepts credit cards, debit cards, check and cash. Any co-pay that your health insurance company requires will be collected by our office staff prior to seeing a physician. for services rendered and/or for your health insurance co-pay, if any.
What should I expect during my first appointment with Pain Management Consultants?
Sign in with our office staff and one of our other health professionals will be with you shortly. Our team will review your New Patient Forms package, and take any medical reports or radiology results that you may have brought with you. We’ll talk about your health and medical history, and perform an examination or consultation. If any new testing is needed on the first day, or in the near future, we’ll give you the prescription order. After testing, we’ll give a diagnosis and options for treatment.
I have an appointment but have to reschedule it. What do I do?
We appreciate knowing well in advance if you must cancel an appointment. Patients who call at least 24 hours in advance of the appointment time can reschedule an appointment at no charge. You must call Pain Management Consultants at (239) 333-1177 to cancel. Please do not e-mail.
I missed my appointment and did not cancel in advance. What now?
We are hopeful that you will never miss an appointment, but we understand things happen. “No show” patients are billed $25 for each office visit missed and $75 for missing a scheduled procedure. These fees will be collected prior to seeing one of our health professionals during your next appointment.
